Early Warning Scores, Explained
NEWS2, MEWS, and their peers turn continuous vitals into a single deterioration signal. How they're built, and where they fail.
The concept
An early-warning score (EWS) assigns points to each vital sign based on how far it deviates from a normal range, sums the points, and maps the total to an action: increase monitoring frequency, call a nurse, activate a rapid-response team. The point of the score is not diagnostic — it is to compress six numbers into one that a busy clinician can act on.
NEWS2 in detail
The UK Royal College of Physicians' National Early Warning Score 2 (2017) is the most widely adopted adult EWS. It scores six parameters, adds 2 points if the patient is on supplemental oxygen, and uses a separate SpO2 scale for patients with hypercapnic respiratory failure.

A total of 5+ (or any single parameter scoring 3) triggers urgent clinical review; 7+ triggers emergency response.
MEWS and other variants
- MEWS (Modified Early Warning Score) — five-parameter US-origin score, still common in EHR-embedded implementations.
- qSOFA — three criteria used to flag suspected sepsis outside the ICU; not a general deterioration score.
- PEWS — pediatric variant, with age-banded thresholds.
- Vendor-specific "acuity scores" — often EHR-derived; validation quality varies widely.
Limitations
- Chronic derangement: a stable COPD patient with resting SpO2 of 90% scores as deteriorating.
- Trend blindness: a single-point score does not capture the direction of change.
- Ceiling effects: patients already in extremis stop rising on the scale.
- Escalation quality: a score without a reliably staffed rapid-response team simply generates alerts.
The current research direction is toward trend-aware, ML-based deterioration models that ingest waveforms as well as periodic vitals — but these remain adjuncts to, not replacements for, the classical scores in most institutions.
